AUTHORIZATION FOR MINOR TO PARTICIPATE IN
Lorain County Community College Network Gaming Association's
BI-WEEKLY LOCAL AREA NETWORK TO BE HELD ON THE 1^st and 3^rd SATURDAYS OF EVERY MONTH
RELEASE OF ALL LIABILITY AND AGREEMENT TO INDEMNIFY AND HOLD HARMLESS

Lorain County Community College (here in referred to as LCCC) and Lorain County Community College Network Gaming Association (here in referred to as LC3NGA) want to provide a safe fun experience for all participants. Individuals of all ages are welcome at this and other LC3NGA events. However any minor(s) under the age of eighteen that wish to participate in this event, both the minor and the minor(s) parent and/or legal guardian must sign this authorization, waiver, release of all liability and agreement to indemnify and hold harmless before you will be permitted to participate.  To help insure the safety of everyone's equipment, The LC3NGA and/or LCCC may record parts of the event with video surveillance. LCCC and/or LC3NGA may take these means to reduce the possibility that someone may leave with a Computer or Monitor they did not arrive with. LC3NGA

We, __________________ (minor's name) and___________________________(parent/legal guardian) (circle one) hereby waive, release and forever discharge LCCC and LC3NGA, its officers, directors, stock holders, employees, staff, volunteers, and members from any and all liability for any personal injuries or property damage that may be sustained as a result of, or during, participation in any LC3NGA events, including any personal injuries or property damage that we believe may have been sustained as a result of the negligence, in whole or in part, of LCCC or LC3NGA. We further agree to indemnify LCCC and LC3NGA, its officers, directors, stock holders, employees, staff, volunteers, and members from any and all claims of any nature whatsoever, including, but not limited to, personal injury and property damage, by any third party or third parties, which they claim are caused, in whole or in part, as a result of any act(s) or conduct, whether unintentional, negligent, or intentional, by one or both of us, which may occur immediately prior to, during, or immediately following any LC3NGA events.

We understand that LCCC and LC3NGA maintains a strict no smoking, no alcohol and no drug policy and we agree not to bring with us to any LC3NGA event or distribute at any LC3NGA event any smoking materials, alcoholic beverages or any illegal drugs. LCCC and LC3NGA hopes that  these precautions will be seen as a benefit rather than an inconvenience. We also understand and acknowledge that no surge protection is provided by LCCC or LC3NGA at this event. We have been urged by LCCC and LC3NGA to provide suitable electrical surge protection for all equipment utilized by us at this session. By affixing our signatures hereto, we agree to assume the risk that our computer, monitor, software or other equipment may be damaged or destroyed by power surges or other types of electrical activity.

We understand that our machine(s) are our personal responsibility and liability, and agree to indemnify and hold harmless LCCC and LC3NGA its officers, board of directors, employees, representatives, volunteers, staff, and members from any claims arising out of, in whole or in part, our use in any way of our computers, monitor, or other computer related equipment or software/programs during any LC3NGA session.

We, __________________ (minor's name) and___________________________(parent/legal guardian) (circle one) also understand that the minor listed above is solely responsible for anything download/uploaded or installed at an LC3NGA event. It is further understood that certain games played at LC3NGA events have been rated for certain age groups and it is our responsibility to know what games are too mature and not intended for minors.  By executing this waiver both the minor and the (parent/legal guardian)(circle one) agree to indemnify and hold harmless LCCC and LC3NGA its officers, board of directors, employees, representatives, volunteers, staff, and members.

LCCC and LC3NGA do not, under any set of circumstances, condone any type of illegal activity whatsoever, including but not limited to the copying of illegal software, MP3's or distribution of pornographic material to minors. By executing this waiver, release, agreement to indemnify and hold harmless, we acknowledge that we understand and agree to accept each of the statements, waivers, releases and agreements contained in this document and by our affixing our signature hereto affirm that we will abide by these policies.
